HEA Foundation
About the Foundation
To meet the critical eye care needs of those less fortunate within Houston and its surrounding communities, the physicians of the Houston Eye Associates formed the Houston Eye Associates Foundation (HEA Foundation) in 1981. They created the HEA Foundation with the belief that everyone should be provided with the most scientifically advanced medical eye care. The physicians of Houston Eye Associates and other medical professionals donate their office and surgical time, and diagnostic skills for those in financial hardship. The Houston Eye Associates Foundation pays for hospital and surgical facility fees, vision-saving medications, and other ancillary expenses, such as prosthetics, and pre- and post-op laboratory and diagnostic testing.
The HEA Foundation is supported solely by the generous charitable donations from individuals, private foundations, corporations and annual fundraising events. The Houston Eye Associates Foundation does not receive government or funding of any other kind.
All HEA Foundation patients are screened to ensure they have exhausted all other resources through their county indigent health programs and from the governments (Medicaid/Medicare). Applicants must also provide complete household financial documentation (most recent completed tax return, last two pay stubs with year to date figures, an employer/family support letter, etc.).

To qualify for Houston Eye Associates Foundation assistance, the following items must be provided:

3) Provide a copy of the completed 2010 1040 income tax form of all working household members.
4) Provide current Year-to-Date pay stub from employer of all working household members.

If you do not have an income tax return, you will need to send proof of income, such as a handwritten letter from your employer or W-2. If you are unemployed and living with family members, please send proof of household income for the family with whom you are living.
