About the Foundation
To meet the critical eye care needs of those less fortunate in Houston, the physicians of the Houston Eye Associates formed the Houston Eye Associates Foundation in 1981. They created the Foundation with the belief that all Houstonians should be provided the most scientifically advanced medical eye care regardless of their ability to pay. The physicians of Houston Eye Associates donate their time, experience, diagnostic and surgical skills and office visits to care for the indigent. The Houston Eye Associates Foundation pays for hospital and surgical facility fees, medications, and other ancillary expenses. The Foundation is supported solely by the charitable donations of individuals, private foundations, corporations and two fundraising events. The Houston Eye Associates Foundation receives no government or other funding of any kind. All Foundation patients are screened in order to insure that there is a valid need for medical assistance through the Foundation and they have been denied assistance through the government and county. They must also provide documentation that verifies their income.

To qualify for Houston Eye Associates Foundation assistance, the following items must be provided:
1) Show proof of currently denied Medicare/Medicaid assistance.

3) Provide a copy of the completed 2009 1040 income tax form of all working household members.
4) Provide current Year-to-Date pay stub from employer of all working household members.

If you do not have an income tax return, you will need to send proof of income, such as a handwritten letter from your employer or W-2. If you are unemployed and living with family members, please send proof of household income for the family with whom you are living.
